JOB DESCRIPTION:

• Perform physical exams and patient observations.
• Record patients' medical histories and symptoms.
• Create patient care plans and contribute to existing ones.
• Order, administer, and analyze diagnostic tests.
• Monitor and operate medical equipment.
• Diagnose health issues and create a plan of care for patients.
• Administer medicine and other treatments.
• Detect changes in patients' health and modify treatment plans, as needed.
• Consult with healthcare professionals and families.
• Train patients and their families to manage and prevent illness and injury.
• Prescribe appropriate pharmacologic and non-pharmacologic treatment modalities.
• Facilitate the patient's transition within and between health care settings.
• Monitor and provider start-up status of clinics.
• Provide coverage to additional clinic locations, as needed and assigned.

• 2+ years of experience in family medicine as a Nurse Practitioner/Physician Assistant.
• Graduate of an accredited school of Nursing/Physician Assistant Program.
• Current Registered PALicensure in State of Texas.
• Current PA Licensure in State of Texas.
• Current Board Certification in adult primary care or family practice specialty area.
• Current Medicare provider enrollment with active PTAN number.
• Current TX Medicaid provider enrollment with active TPI number.
• Current provider card in Basic Life Support (BLS).
• Current provider card in Advanced Cardiac Life Support (ACLS), or willingness to obtain upon hire.
• Current DEA License, or ability to obtain DEA license upon hire.
• Experience in geriatric care preferred.
• Previous experience with Athena EMR, preferred.

• Strong computer skills.
